Anticipatory Care

Anticipatory care (AC) is a Long-Term Plan commitment focused on provision of proactive care in the community for multimorbid and frail individuals who would benefit most from integrated evidence-based care.

Viaduct PCN’s Care Coordinators visit patients with moderate to sever frailty who have a high a number of A&E or GP appointments to see how we can help. Our team aim to improve patients lives and reduce the risk of patients falls wether thats installing a new hand rail or making other home improvements to prevent falls. We are the only PCN in Huddersfield to offer this service personalised service.
