Hi, my name is Claire Hill and I am one of the Social Prescribers for the Viaduct Primary Care Network.
About My Role
I am the Mental Health Social Prescribing link worker, I can offer one to one support to Anyone who would like to talk to someone about there Mental health, I will listen to what’s important to you and offer support based around you as an individual.
Our health and wellbeing can be effected for many different reasons from social, economic and environmental factors and can effect how we feel. It can help to share how we feel with someone else.

I can support you with a range of support from managing your health to connecting to support groups, housing, finances, employment and support to reduce isolation and connect you to services.
