About My Role

My role is to visit patients in their homes who would benefit from support through setting goals that will improve their health and wellbeing. I will work with patients to find out what Matters to them and support them to develop their own care plan to help them achieve their goals.

To support patients to achieve their goal I will work with them  to identify appropriate organisations and services available to support them to achieve their goals and make onward referrals where required.
